As part of your tribal gaming license application process, you are required to bring the following documents with you for the following position license classifications:
Primary Management Officials (PMO); Key Employee (KEY); and Non-Key Employees:
- A completed gaming license application
- Valid State Driver's License/ID Card
- Social Security Card
- Certificate of Birth
- High School Diploma
- Copy of any post-secondary education degrees, professional license, certificates, and/or official transcripts
If Applicable
- Tribal Enrollment Cart/Certificate
- Military ID Card/DD214/DD215
- Certificate of Naturalization
- Alien Registration Card
- Valid Passport
- Court Documentation - provide copies of any final court disposition and/or court proceedings pertaining to any criminal/civil case disclosed on the application
PMO/KEY Employees
- IRS Taxes Filed from past three (3) years
NON-KEY Employees
- W-2s from past three (3) years
Burden of Proof
An applicant is seeking the granting of privilege. The burden of proving the applicant's qualifications is at all times on the applicant.
