Notice To Licensing Applicants

As part of your tribal gaming license application process, you are required to bring the following documents with you for the following position license classifications:

Primary Management Officials (PMO); Key Employee (KEY); and Non-Key Employees:

  • A completed gaming license application
  • Valid State Driver's License/ID Card
  • Social Security Card
  • Certificate of Birth
  • High School Diploma
  • Copy of any post-secondary education degrees, professional license, certificates, and/or official transcripts

If Applicable

  • Tribal Enrollment Cart/Certificate
  • Military ID Card/DD214/DD215
  • Certificate of Naturalization
  • Alien Registration Card
  • Valid Passport
  • Court Documentation - provide copies of any final court disposition and/or court proceedings pertaining to any criminal/civil case disclosed on the application

PMO/KEY Employees

  • IRS Taxes Filed from past three (3) years

NON-KEY Employees

  • W-2s from past three (3) years

Burden of Proof

An applicant is seeking the granting of privilege. The burden of proving the applicant's qualifications is at all times on the applicant.
